Let's face it...we are missing in-person connection! Rev. Christy will be hosting small group sessions in South Charlotte in one-hour increments on October 1st. Each session will be limited to less than 15 people. We will wear masks, gather outside, stay socially distanced yet be able to be together and connect. Liability and Assumption of Risk and Waivers of Liability Relating to Coronavirus/COVID-19 *

Student Liability Waiver: My online registration acknowledges that I alone am responsible to decide whether to attend this Community Gathering with Carolina Center for Spiritual Awakening. I hereby release any teacher and this facility from all liability, negligence, or other claims arising from or in any way connected with my participation. My registration is acknowledgment and binding to this liability waiver from this day forth.

Assumption of Risk and Waiver of Liability Relating to Coronavirus/COVID-19: The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people in indoor spaces and large groups in outdoor space.

Carolina Center for Spiritual Awakening has put in place preventative measures to reduce the spread of COVID-19 such as: -

Sit/Stand  6’ apart -

Bathroom access, one at a time, cleaned after each class -

Respect others space (social distancing) -

If you are sick or have been around sick individuals please stay home -

You MUST bring your own chairs

However, we cannot guarantee that you will not become infected with COVID-19.

By registering and attending this event, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending this event and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, The Carolina Center for Spiritual Awakening employees, volunteers, and participants. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with attendance at Carolina Center for Spiritual Awakening or participation. I hereby release, covenant not to sue, discharge, and hold harmless Carolina Center for Spiritual Awakening, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs, or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of CCSA, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any event.
